Acute respiratory distress syndrome
Acute respiratory distress syndrome (ARDS) happens when fluid gathers up in the small, elastic air sacs (alveoli) in your lungs. The fluid prevents enough air from filling your lungs, ensuring that less oxygen enters your bloodstream. This absolves the organs of the oxygen they need to operate. Usually, ARDS develops in persons that are either seriously ill or with serious injuries. Extreme difficulty breathing occurs typically within a few minutes to a few weeks after the crystallizing inflammation or illness, the critical sign of ARDS.
Mr. Nguyen may develop some problems caused by the ventilator itself or by circumstances that are much more likely to occur while you’re on a ventilator. The breathing machine could release bacteria that damage the tiny air membranes in the walls of your lungs in your airway. Plus, it is more difficult for the tube to cough out particles that could infuriate the lungs and lead to infection. It can be dangerous for your lungs to have too much oxygen in the mixture for too long. It can injure your lung tissue unless the force or amount of oxygen is too much or if your lungs are too small. This ventilator-associated lung injury (VALI) is what your doctor would call.

Potential interventions include intubation avoidance, sedation minimization, combined frequent spontaneous arousal and breathing tests, early exercise and responsiveness, ventilation of low tidal volume, progressive fluid control, and requirements for conservative blood transfusion.

- Minimize exposure to ventilators.
- Provide definitive treatment for oral hygiene.
- Coordinate sub-glottic suction treatment.
- Ensure high placement and promote versatility.
- Ensure ample staffing.

If I were orienting in the ICU and the nurse am working with is not implementing the VAP interventions, I would test the nurse’s current awareness about VAP intervention and teach him/her about the meaning of VAP prevention. Then I would educate her about methods for VAP prevention and also inform about Packages of VAP prevention.
