Goals
One of my main goals for this semester is to be a team player. Collaborating with others is vital as it helps one learn a lot along the way and interact with other minds where they help build your confidence. To achieve my goal of being among the most valuable employees, I won’t do it alone. I need to appreciate other people and utilize the input they have for the same purpose.
Objectives in the cognitive domain
- Remember – I will be able to identify the phenomena under study through recalling information acquired throughout the learning experience and infer them accordingly.
- Application –I will be able to utilize patients’ information to develop a comprehensive understanding of the situation vital for making decisions.
- Analysis-I will be able to analyze patient information, breaking it down into parts for better understanding. This will enable me to differentiate between one phenomenon and another to give the best recommendations for the patient.
Objectives in the psychomotor domain
- Observation – I will be able to display observational skills that require learning from other experienced people through watching and relating the action to the information at hand. This will make it easier to grasp and understand certain situations better.
- Practising – I will handle situations by automating the sequence and making it smooth through practising over and over. This will help perfect my skills in handling patients without fear of making a mistake.
- Adapting –I will be able to adapt to the environment and the system by fine-tuning my skills to better fit in the system without a hitch and offer solutions to different situations.
Objectives in the Affective domain
- Receiving – I will attend to the patient’s needs by being aware of the emotional factors, listening attentively to the patient to be able to gather the vital information. I will employ empathy and be receptive and being aware of other existing ideas.
- Responding-I will comply with the procedures and processes involved. Being aware of a phenomenon and responding to it accordingly.
- Valuing –I will be able to accept the information received, especially patient information and appreciate it. I will be able to take responsibility for the improvement of the working conditions using the available information.
The problem is the patients with High Blood Pressure and ways to help them cope with their medication and live healthy. Hypertension increases as one age, and it mostly affects older people who are 55 years and above. Creating awareness through the educational program is a vital tool that will help save many lives lost due to a heart attack through high blood pressure.
The program targets the middle-aged- adults (36-55) most of who continue to smoke and take large quantities of alcohol which are among the contributing factors to hypertension especially for those who already have high blood pressure. The program will be geared towards ensuring the patients are taught the importance of quitting smoking, reducing alcohol intake, cutting back on caffeine, reducing sodium in the diet, regular exercises and stress reduction.
The expected change is to be made by a collaboration of both the patients and the stakeholders. This, therefore, means that for the program to be successful, everyone has to play their part and be disciplined in accomplishing the mission. By the end of the ten weeks, all the participants will be able to know the risk factors involved in HBP, will be exercising at least 30 minutes every day, adhere to their medication, check their diet and at least half of those smoking and taking alcohol would have quit and or reduce their intake.
I will talk to the stakeholders concerned with the HBP disease which include the medical assistants, patient educators and social workers who will help provide with a more dynamic and robust inspection of hypertension care and help the patients better understand what they are required to do to get the best for their health. With the available resource at my disposal, I will achieve the set goal.
The survey that I will develop in the next seven weeks will enable the participants to answer at least 75% of the questions regarding hypertension correctly. The program will target changing the patients’ beliefs, norms and habits regarding their lifestyles and medication adherence. This will help them change their view on hypertension and realize that they can be able to control their blood pressure by following the laid down guidelines and reduce the chances of having a heart attack.
