Capstone Change Project Evaluation Plan
The primary objective of the proposed intervention – culturally competent diabetes self-management education – is to reduce disparities in HbA1c in Harvey’s black community. The capstone change project intends to reduce HbA1c levels from the highs of 9% and above to below 5.7% within three months. The implementation of the intervention is expected to enlist the input of several stakeholders ranging from patients to relevant government agencies. A combination of the financial and material resources will be required to implement the intervention successfully. It is evident that the success of the proposed intervention in addressing diabetes health care disparities in Harvey’s black community will depend on the interplay of various factors.Nonetheless, proper evaluation mechanisms will be required to help monitor the overall progress of the change project and make relevant corrective measures whenever the need arises.
The success or the progress of the change process will be evaluated using a combination of process, impact, and outcome evaluation methods. The Process evaluation method will crucial in analyzing the impact of the intervention on the individual and collective outcomes of the targeted patient population. The method will also be crucial in analyzing stakeholders’ level of satisfaction and experience with various aspects of the change process. On the other hand, the impact evaluation will measure the short term effects of the culturally competent diabetes self-management education on HbA1c levels in the community. Lastly, the outcome evaluation will be crucial in assessing whether the overall objectives of the change program have been achieved, challenges posed by the eternal factor, and the long term effects of the intervention on the prevalence and incidence of type 2 diabetes in the community.
