Chronic Kidney Failure
Mr. Rojas should be aware of the criteria he should meet to be placed in the kidney transplant list. One of the requirements is that he should not be of so much advanced age; Mr. Rojas is just 49 years, which qualifies him for the procedure. People with severe cardiac disease are never allowed to undergo kidney transplant due to increased chances of complications and even death. If the patient is also recently treated for cancer, then they are not suitable candidates for a kidney transplant. The presence of poorly controlled mental illness such as dementia is another contraindication. If the patient has been actively abusing drugs or alcohol, then it is an ultimate contraindication for a kidney transplant. Also, patients with BMI above 40, severe pulmonary hypertension, history of not complying with medications may make them not go through kidney transplant (Tampa General Hospital, 2020). Apart from the mentioned factors, any other factor that undermines the patient’s well-being during surgery and after the surgery are considered part of the criteria.
Mr. Rojas has three kidney transplant options: the living donor, the deceased-donor, and the preemptive kidney transplants (Mayo Clinic, 2020).
Recommendations to Mr. Rojas on taking medications: he should know that he will use his immunosuppressant medications and prophylactic antibiotics for a lifetime; to help against organ rejection and reduce the risk of infections, respectively.
Alternative kidney replacement therapies to educate Mr. Rojas include peritoneal dialysis and comfort care. Peritoneal dialysis gives the patient freedom and flexibility, reduces hospital visits, and reserves the remaining kidney function. Its disadvantage is that it leads to the development of hyperglycemia and increased weight gain (Atsuchi, 2017). Comfort care helps prepare the patient and family for the impending death and reduces hospital costs, but it may instill hopelessness.
