The patient will ensure they clean the wound regularly, as instructed by the Nurse. The patient will monitor his glucose level and regulate it as prescribed. The patient wound will decrease in size and eventually heal. The patient will maintain their blood sugar at the normal prescribed level by the Nurse.

The nurse should monitor the wound for any signs of Infection or inflammation of the wound.

Check the Glucose and blood pressure of the patient every 3 hours while in the hospital.

The nurse will educate the patient on ways the ensure the wound is healed and proper care techniques of the glucose level.

The strategies I used for the topic included creating a written diet plan for the patients, Physical demonstrations on how to inject themselves with insulin, monitor their body temperature, measure glucose intake and measure blood pressure in the body.

| I determined if the client/ Family members have learned the information By using a hypothetical case of a patient who had diabetes. Asking them questions and answers of the care plan to follow, the diet to eat, interval on the intake of medication, and how to administer insulin injections and drugs when needed. |
