And also, the subscription plan provides a personal coach and many more features. The app has a lot of information that can be used in treating hypertension. It has a vast variety of food options to choose from, which are healthy in diet and digestion. Determine and Evaluate Success (3)
- Conduct an assessment for direct questioning to ascertain the information that the patient has misinformation or opportunity areas. These include the ability to realize the benefits associated with the app.
- Conduct a demonstration as well as an explanation regarding the app and the necessary information needed. Upon completion, the patient has to complete a teach-back method as a way of demonstrating the ability to use the app effectively. This includes journal accessibility of health reports, statistics interpretation, and meaning of the information within the resource.
- A final evaluation is needed to ascertain the patient’s level of education regarding the app and the success of the teachings.
