Dear Professor Tapalla,
I am writing a formal request that you change my grade in Nursing 101 for the spring semester (Months of the semester) from a D to a C. I understand why you gave me a D grade, but I hope you will amend it because I was unable to write the exam due to certain circumstances beyond my control. I performed well throughout the coursework since I received an A on every class assignment. However, since I could not write the final exam, I receive an F, and my final grade was a D.
I could not take the exam because I was admitted to the hospital with stomach cramps as a result of food poisoning. I was under care the day which the exam was held. I am confident that if I could have sat for the exam, I would have scored a B or C+ grade based on my performance in other courses. I would be glad to take the exam if you would agree to give it to me privately at any time. I will appreciate a lot if we can meet at any time that is convenient for you so that we can discuss the possibility of raising my grade.
