Ever nurse-patient interaction begins with creating a good rapport to put patients at ease and to communicate freely. Building a comprehensive history requires adequate data collection on the several aspects of patient medical history, and lifestyles (Lebanon). The questions seek to grasp all these aspects, which are within the healthcare contexts. The medical, family, socioeconomic, and gynecological history in women is crucial in determining and treating chronic diseases such as cancer, diabetes, hypertension, and kidney diseases. The Security Risk Assessment Tool (SRAT) was designed to help health professionals meet HIPAA guidelines of patient security and confidentiality (Landoll & Landoll, 2015). It has features such as progress tracker, threats and vulnerability and asset tracking, and detailed patient reports applicable in assessing health risks.

Thank you for the educative post. Building a comprehensive health history vary depending on the patient’s age, gender, condition, and culture. To obtain adequate data, the health provider must respect personal space, patient feelings, and privacy, and create a conducive environment during the interview. The communication techniques used in the interview process with the pregnant teenager include open-ended questions, active listening, and non-verbal cues (Fleischer, 2019). Since most of the teenage pregnancies are unplanned, the nurse should prove comfort, empathy, and reassurance. Others may view the pregnancy as a way out of poverty and abuse, but still face stigma, shame, and unacceptance. Therefore, the healthcare provider should be keen in identifying patient feelings, actions, and perceptions regarding the pregnancy. The question you asked is in accordance with the pregnancy guide and will not elicit any suspicions. The Edinburgh Postnatal Depression Scale is an effective tool used in assessing the risk of postpartum hypertension (Cox & Holden, 2018). It begins as early as the second trimester to identify symptoms and offer early management. It consists of ten questions aimed at assessing patient interests, sleep patterns, perception of pregnancy, and signs of self-harm; thus, it is suitable in this case.
