Robin James of +1(312) 762-5829, 45 years male who hails from Chicago was referred to our facility. Robin is married with two children. He was born on 4th January 1975. He works as an engineer in one of the companies in Chicago. He works with Primera Engineers.  He was interviewed on 3rd January the year 2020. Robin has a master degree in engineering from Chicago University. The can be reached at any time with the contact mentioned above for any emergency purposes.  All the information provided is the accurate information which Robin provided in our facilities during his visit.

Reason for referral

Robin was referred for further evaluation by the community health worker. Robin had visited the community health centre for counselling. He lost his parents one year ago and was undergoing trauma as a result of such loss. This had been noticed by his wife, who accompanied him to our facility.  In addition to the stress of having lost the parents through an accident, Robin has been battling with other medical conditions like diabetes and chronic illness. Thus, the wife narrated all situations which her husband had experiencing. This had made his condition worsen as a result of the agony of the death of his parents.

Current situation and functioning

From the past medical history, Robin got involved in an accident on his way to work. And he had a traumatic brain injury. He was treated and discharged. However, due to the stress of losing his parents in an accident, his diabetic condition as well as chronic problems. Robin faces multiple health conditions which requires much care. Currently, he has been seen depressed all the times at his workplace.  Due to his cooperation to take part in treatment, it is expected that his condition would improve now that he made a step further to visit the care facility with the aid of his wife. He is open-minded persons and reveals everything which bedevils him. This is imperative as most patients who visit the care facility fails to give appropriate information on their health status.  However, the case chronic case of the Robin seems to be scared as it was in its advanced stage.

Relative medical history

For the prior period, Robin has been suffering from chronic illness. An issue which saw him made frequent visits to the healthcare institution accompanied by his wife. Chronic disease of Robin dates from an early age.  So, apart from stress, chronic illness is the pronounced current illness which has been recorded by the patient.  Again, the issue of brain injury was reported. Robin had got into an accident leading to his brain injury. A case, which he got treated and discharged; however, with an eruption of stress experienced, Robin had to be assessed on the same problem.

Within many hospitals insisting on inter-professional associations, medical team assessing got empowered with the various recommendations from the referral centre regarding the care of a patient. For the prior period, it was the doctor who made essential decisions concerning how patients were treated. However, this has changed due to the introduction of inter-professional association. For instance, in treating chronic, team members had a different acquaintance come together to offer their skills. In Robin’s case, professional association assisted brain injury treatment incorporated different professionals from the hospital. The neurologists, RNs, cardiologist and surgery experts composed a team who made his treatment a success.

Psychiatric Treatment History

The mental problem arising from the trauma Robin had faced after losing his parents made him close communication gaps he used to have. Robin had attended various counselling programs to help improve his stress conditions. Before the adoption of psychiatric treatment, medical expert only checked on the patients’ charts to assess procedures and chronology (Borg & Pålshaugen, 2019). Independent working led to miscommunication or missed symptoms concerning the needs of a patient. The inter-professional team could work together in developing more communicative environments to address the problems faced by Robin. Through collaboration, Robin interacted at personnel grounds, sharing of notions regarding the past treatment of her husband as he could not give appropriate information due to stressful sentiments. Also, the wife worked together towards maintaining care continuity form various medical assistance they had sought for his conditions.

Family History

As mentioned earlier, Robin is a married man with two children, and they live in Chicago, which happened to be his working place. He had a good relationship with family members. He has three brothers who are trained teacher, nurse while one is a business person. He has two sisters, who are both registered nurses. He is the firstborn in a family of five. However, in the turn of an event after losing his parents through an accident whom he adored much, things turned as he commenced undergoing stressful events remembering the love he had for them. Robin has never shed off the lovely memories he had with his parents at his childhood, adolescent and a time when he now a family man as well. The memories seem to haunt him.

Social and development history

An in-depth interview of Robin revealed that he had earlier had a sour relationship with his first wife, which led to their separation. It was his parents, who are not in the equation that used to encourage him on better ways to have a good marital life. His parent was the essential pillar in his current relationship as narrated by the wife. Robin confided a lot to his parents.

Educational and occupational history

Robin completed his education under the assistance of his parents. His educational life has been revolving around Chicago. He started schooling in Chicago from Kindergarten and up to university level where he studied engineering.  He is a travelled person, occupationally. His employer, Primera often sends his out to handle technical projects even outside the country. This has improved his job performance, which earned him a promotion to a senior position within the firm.  However, with the current predicament on the loss of his parent as a result of the accident, Robin has been off and on from his work station as a result of the health problem. This has made his wife worry and starts walking with him, looking for medical solutions for his health problem.

Cultural influence

Having travelled to various places due to his occupation, Robin has worked with a diverse population. This has influenced how he view life events. He often portrays himself as a man who is capable of mingling with people without infringing their rights.
