Date: ______ Time Name/Course__________________________________
Assessment: Primary and secondary data
| Physical examination of the patient indicates that he has high pressure=163/91mm Hg, High pulse rate=82 beats/min, High cholesterol levels=239mm/dl, Low levels of HDL=28mg/dL, overweight=261IL, and High levels of LDL=181mg/dL. Furthermore, the patient feels pain in the right hip and right leg calf pain when walking (claudication). |
Nursing Diagnosis | Peripheral artery disease (PAD) |
Intervention | The patient should be medicated on a daily dose of aspirin. Aspirin is considered as acetylsalicylic acid and can help the patient to get relieved from the clots which form in the blood. Also, the patient should receive antiplatelet therapy with a prescription of clopidogrel 75mg daily. The clopidogrel is recommended because it prevents platelet clots from forming in the blood vessels. The patient should also be under daily statin therapy. Statin medication is lip-lowering medications and will reduce the patient’s risk of developing cardiovascular disease. The patient should receive food therapy in order to teach him about eating a healthy diet concerning his disease. |
Evaluation | The patient will be able to walk for at least three city blocks without feeling pain on his right leg calf and right hip and gradually increase his pace and duration of the exercise. After the end of the treatment, the patient will be able to apply some dietary measures to help him reduce his weight. As the patients continue to observe strict diet and body weight, his blood pressure, pulse rate, and density lipoprotein level will gradually return to optimal levels |
Revisions | The patient should have another coronary artery bypass graft (CABG) operation to restore normal blood flow to an obstructed coronary artery that is causing pains on his right left calf and right hip. |
