TREATMENT PLAN – INSTRUCTIONS
Dimensional Treatment Plan As you have discovered from your clinical and educational experiences diagnosing and treating mental illnesses is, as the saying goes, both a science and an art. As a result, this process requires both skill and insight. Thus, this a unique opportunity to provide a diagnosis based on the presenting problems of a client with whom you are working, and subsequently develop a treatment plan that takes into account all of the client’s issues, including the requisite service and support needed to create positive change in the client’s life. In doing so, you engage in both the science and the art, by practicing your dimensional diagnostic skills, reflecting on the insights you gain from the diagnostic process, and applying this knowledge to a plan that promotes client welfare and positive change. To prepare for this assignment: (3-4 pages in length) For this assignment, think about a client with whom you are currently working. Consider: his or her reason for seeking counseling services, current symptoms and behavior, history of problem, impairments in life functioning, psychological/psychiatric history, current risk and safety concerns, current and/or previous medication, current substance use and/or substance use history, family background, personal history, mental status, and any additional miscellaneous information that may be an influence to the client’s well being Based on this informational input write out your clinical impression of the client’s problems and symptoms using a narrative format (similar to a case study format). Include your observations about how the client looks and their behavior or attitude, basically any and all mental status concerns you observe during your interview assessment of the client. Review these symptoms and determine a diagnosis based on the criteria presented by the client. Consider a diagnosis and provide your thoughts about why you chose that diagnosis. Consider any other possible diagnosis and why that diagnosis did not fit your client’s symptoms. Create your treatment plan for this client based on the objective that you see should be address for the client to heal and get better. Set long term goals, and short terms goals to meet those objectives. Determine at least 2-3 objectives and their goals both long term and short term. Provide a prognosis for your client considering that your client follows and meet the goals set. Discuss theoretical orientation you will use, and any other interventions you think would benefit the client. Include in your writings at least one evidence-based intervention strategy you plan to use with your client and justify the strategy that you will use with your client. Format for this paper: Provide your narrative of this client in paragraph format using appropriate titles and sub-titles to present your discussion of client information.
